This form is used to ensure a protected evaluation of student candidates for the Radiographic Technology Program. Its use is to insure that each candidate submits an unaltered evaluation of their skills, work ethic, and professional performance and that each student receive an equitable evaluation by the reviewing committee. Personal letters of recommendation will not be accepted.
