If you are in the greater Denver Metro area you will be expected to come to campus to complete this form. If you live outside of the Denver Metro area, you can mail the form, but please be sure to provide all necessary requirements. A notary will need to complete their section on the form itself, and you'll need to include a copy of your unexpired, government-issued, photo ID.
