The Honors Program Completion Form can be completed by students who have completed a minimum of 15 credit hours AND have maintained a cumulative grade point average of 3.5 or higher.

Upon completingthe Honors Program, you will be eligible to receive formal recognition. Please fill out and complete this form and turn it intoChase Hart, Honors Coordinator. This form has a deadline eachsemester, please read it fully before completing.
