This form is used to acknowledge that a P-Card holder has received a copy of the P-Card manual, and states that he/she understands and will abide by the P-Card rules governing the P-Card use. This form needs to be signed by the P-Card holder and the P-Card administrator. The form will be maintained by the P-Card administrator.
