This form is for the collection of Department of Homeland Security or other U.S. citizenship/nationality documents from students unable to present their documents in person. The student will submit this form along with requested citizenship/nationality documents and a valid government-issued photo ID card bearing their portrait. This form serves as self-certification all attached documents and government-issued ID are the true, exact and complete copies of originals issued to the student.
