The purpose of this form is to communication to adjunct faculty his/her assigned courses for the appropriate term. Adjunct instructors are responsible for completing the form and returning it to his/her respective department chair. Please use the late hire version if adjuncts are assigned courses and the 21 days deadline does not apply.
